MS4 compliance inspection

The Municipal Separate Storm Sewer System (MS4) inspection report is used to assist State and National Pollutant Discharge Elimination System (NPDES) staff in evaluating the compliance and effectiveness of MS4 programs in order to cooperate with state requirements under the  U.S. Clean Water Act . There is an Office visit and a Field visit version of the report. The report is usually completed digitally and submitted to the Compliance Assurance section.

These survey design choices help users save time and fill in only the necessary fields:

  • Autocompletethe municipality and county names are automatically filled in based on the Mailing address
  • Relevantbased on whether the user chooses Office visit or Field visit, only the questions related to the selected option will display
  • Grid themequestions are organized in a grid allowing them to fit efficiently within the screen space

Export a formatted report

After completing the survey, a report template can be created in the state's required format. The report template can be customized depending on the required format.

Point-of-Entry-Treatment (POET) Systems

 The Safe Drinking Water Act  (SDWA) aims to protect drinking water and strengthen public health protection. The  National Primary Drinking Water Regulations  (NPDWR) are responsible for setting primary standards and treatment techniques that apply to public water systems. Both of these regulations aim to protect public health and limit the levels of contaminants in drinking water.

One way to support safer drinking water is with Point-of-Entry-Treatment (POET) water filtration systems. A POET can be connected to the plumbing of a home or business as a way to treat water for direct consumption, such as drinking or cooking, and to achieve compliance with maximum contaminant levels (MCLs) for state and federal drinking water regulations.

This POET survey enables documentation of the wells that currently supply water, the condition of the wells, the plumbing layout, as well as water sampling details and observations at various stages of water flow. Once submitted, this information can be compiled into a report template and used to support residential well-sampling programs and environmental reporting to stakeholders.

  • Autocomplete— the x-coordinate and y-coordinate are automatically filled based on the location
  • Relevant— 'please explain' fields only appear when an answer is selected that requires an explanation
  • Signature—a signature is written in the survey and exported to the report

The tailgate safety briefing survey enables the documentation of safety topics discussed during the briefing including weather conditions and hazard safety measures. This helps employers to ensure employees are aware of hazards and can reduce the risk of accidents in the workplace.

These survey design choices help automate the process and enhance the visual appearance of the survey:

  • Attendance summarya summary with the attendee's names, company, and time spent on site is automatically populated
  • Calculated time—the time spent on site is calculated based on the Time in and Time out questions
  • Likert scale—temperature and cloud cover can be selected through a slider bar

Outcrop mapping

The outcrop mapping survey is used to record field observations and map an outcrop. The survey collects data including geological units, dip, annotated maps, geological profile sketches, and photos.
