Reporting tax data
The State of Alaska Office of the State Assessor provides reporting form templates for municipalities to use to submit end-of-year reports such as the Municipal Tax Report. These forms are provided in PDF format and must be digitally submitted by the listed due date to the Office of the State Assessor.
Project overview
The Survey123 Municipal Tax survey collects all the data necessary to fill out the State Assessor form. It takes advantage of several Survey123 features to simplify data entry.
Streamlining data entry
The following survey design choices reduce errors and data entry time:
1. Autocomplete—The municipality name is automatically completed based on a choice list of Alaska municipalities.
2. Calculations—Fields like Total Sales Tax Revenue are automatically calculated based on the responses to other fields.
3. No map—Not all surveys need to include a map. This is an example of a survey that does not require any geospatial data to be entered by the user.
4. Grid theme—The grid theme allows questions to be organized in a grid format. This allows form authors to use the screen real estate as efficiently as possible and organize large quantities of questions in a way that is easy to read.
Export a formatted report
Creating a report template allows municipalities to export the survey responses into the format required by the state. The report template is customizable and can be altered if the required format changes from one year to the next.

It is also possible to export a local Excel file containing all the survey responses.
