CDPH Mpox Treatment Providers Application

Step 1:

• Option 1: Type in an address or location in the “Find address or place” bar. You can use:
  • The exact address (for example, 900 Lombard St, San Francisco, CA, 94133) or
  • Zip code (for example, 91942) or
  • A town, city, or county name.

• Option 2: Click or tap directly on the map.

If you want to start over, click or tap the "Clear search location" on the top of the map.

Step 2:

• A list of the mpox treatment providers in the area will appear.
  • The list will have important information about the providers, such as the address, phone number, website, and other information.
  • The closest provider will appear first on the list.
• Point your mouse at any provider on the list to see its location on the map.
• If you click on the dot on the map, it will highlight the provider on the list.

If there is no providers list (blank white), there are no available providers within the selected distance. Follow Step 3 to adjust the distance.

Step 3:

• Find the distance slider (right below the address bar) at the top of the page.
• Drag and slide the distance slider left or right.
• The initial setting is 5 miles, meaning it will search within a 5-mile distance from the selected location.
• You can choose from a 0 to 50 miles distance from your location.
